[KinoSearch] Possible Phrase Query Bug

Thanks for the detailed bug report Matthew.   I've been playing with
phrase scoring recently, so I'll take a look at this.  I don't have a
quad-core Xeon to try it out on :) but I'll see if I can reproduce it
on what I've got and report back tonight.
